Voluntary Group Dental Coverage

Dental benefits will be administered buy Humana and will allow you and/or your dependents the freedom to receive services from the provider of your choice.

ARM pays 50% of the associate cost.

Calendar year deductible per person - $50 (3x per family)
Calendar year maximum - $1,000 or $1,500

Preventive Services - 100% after $50 deductible

Oral Examinations
X-Rays
Cleaning
Topical fluoride treatment (through age 14)
Sealants (through age 14)

Basic Services - 80% after $50 deductible

Space maintainers (through age 14)
Emergency exams and palliative care for pain
Oral surgery
Routine extractions
Non-cast prefabricated stainless steel crowns
Partial or complete denture repairs
Amalgam, composite filings
Thumb sucking and harmful habit appliances (through age 14)

Major services - 50% after deductible
These services are covered after 12-month waiting period

Endodontics (root canals)
Periodontics
Crowns
Inlays and onlays
Removable or fixed bridgework
Partial or complete dentures
Denture relines / rebases
